I believe this is simply a bug. I saved both mausoleums in the mountaintops of the giants for after I beat the final boss as well as Malenia. I also copied my save to allow me to replay as necessary. I am able to duplicate a remembrance at only one of the two mausoleums. To get to the mountaintops you have to beat Morgot, which will also require you to beat the draconic tree sentinel outside of the capital, and Gold Shade Godfrey to get to Morgot.
